Book a routine 10 minute appointment with your doctor, our nurse practitioner (who can do most of what a doctor can do) or blood clinic nurse. For more complex appointments such as medical or minor operations you will still need to phone and discuss with a receptionist.

Simply email our admin team on postmaster@gp-j81613.nhs.uk and they will let you have your registration information. Please provide them with your full name, date of birth and address.

Then use this registration information to create your account and you will be ready and able to use our on-line services.
