They are a team of Doctors and Nurse Practitioners who provide proactive care services for member practices across the Weymouth locality. They look after Care Home Residents and some frail elderly in the community. Weymouth Elderly Care Service is a scheme developed by the Weymouth and Portland General Practice Federation (Two Harbours Healthcare).
